Health Department Receives Grant To Fund Diabetes Care Program
(Providence-AP) -- The state Department of Health has received a grant to improve the quality of diabetes care in Rhode Island.
The 100-thousand dollar grant from the Robert Wood Johnson Foundation establishes the Rhode Island Chronic Care Collaborative.
Through the collaborative program, ten doctors and their staff will work together for one year to test a series of changes in the delivery of diabetes care.
The doctors will use electronic chronic disease registries to monitor patients' needs and progress.
Health officials say the program is a chance to improve coordination of care.
